Client Acknowledgements & Cancelation Policy:

Your intake and Psychotherapy appointment can not begin until you have submitted all electronic consents and agreements.

I will need insurance card or info, date of birth of client requesting appointment and a valid email address to book appointments.

 

I require at least 72 hours to verify insurance coverage.private pay must be made 48 hours before appointment or will automatically be cancelled.

 

Things come up, and sometimes you will have to cancel your appointment. If you do cancel your appointment, Please do so 24 hours in advance.

 

Due to the nature of my schedule and availability we must charge a fee for full service for any appointment that is not kept by the client or is canceled by the client within the 24 hour time frame leading to the time of the scheduled  session. For appointments not canceled on time, a full fee of your scheduled appointment will be charged to your card on file or billed to your insurance.
